Preparing for your USCIS exam

If you're applying for a U.S. green card, the immigration medical exam is one of the required steps on your journey. It's completed by a designated Civil Surgeon authorized by U.S. Citizenship and Immigration Services (USCIS), who documents the results on Form I-693.

At The Well MD in Greer, SC, we perform these exams every week — and the visits that go most smoothly are the ones where patients come prepared. A little preparation can help you avoid repeat visits and unnecessary delays in your application.

Your Checklist

What to bring to your appointment

A valid photo ID

A government-issued photo identification such as a passport or driver’s license.

Form I-693

If you already have it. If not, we provide the form at the office.

Vaccination records

Any proof of prior vaccinations or immunizations you have — this can save you from repeating vaccines.

Medical history & medications

A list of your current medications and any relevant medical history.

Payment

Your fee is due at the time of the visit. We accept cash, debit and credit cards.

An interpreter (if needed)

We speak several languages at the office, but you’re welcome to bring an interpreter if it helps you feel comfortable.

What to expect during the exam

Your visit is designed to meet USCIS requirements and typically includes:

  • A complete physical examination
  • A review of your vaccination records
  • Any required immunizations (if needed)
  • Tuberculosis (TB) screening
  • Testing for gonorrhea and syphilis

A helpful tip: we generally recommend scheduling your exam shortly before you submit your I-485 application, and bringing any vaccination records you already have — it can spare you from repeating immunizations you've already received.
