Save time! We will send you a form to fill out before your appointment. You'll be asked about:

  • Countries & cities you're visiting
  • Purpose of travel
  • Planned activities (e.g. safaris, rural stays)
  • Medical history and current medications
  • Previous vaccinations & any allergies
